WHAT YOU WILL NEED TO BRING:

• Current income of all family members on the application (copy of last year's Federal Income Tax return, W-2 form, two current pay stubs, etc.)
• Legal residency or immigration documents
• Copy of SSN (only if you are applying for coverage) and DOB for each family member in the household
